Descreve o perfil epidemiológico de mortalidade do Hospital de Urgências de Goiás, bem como a avaliação das revisões de óbitos realizados no mesmo período pela comissão de óbito do hospital e investigações de óbitos. Trata-se de um estudo descritivo de abordagem quantitativa, realizado a partir do registro de óbitos e investigações de óbitos realizados pelo Núcleo Hospitalar de Epidemiologia do Hospital de Urgências de Goiás


It describes the epidemiological mortality profile of Hospital d Urgênciasde Goiás, as well as the evaluation of death reviews carried out in the same period by the hospital's death committee and death investigations. This is a descriptive study with a quantitative approach, carried out based on the registration of deaths and death investigations carried out by the Hospital Epidemiology Center of the Hospital de UrgÊncias de Goiás

Introducción: la información sobre las causas de muerte es de gran importancia tanto para los países como para las instituciones sanitarias, en la medida en que contribuye a la evaluación y el seguimiento del estado de salud de la población y a la planificación de intervenciones sanitarias. El objetivo del estudio fue evaluar la proporción de causas de muerte mal definidas e imprecisas y su relación con el día de la semana y período lectivo de médicos residentes en el Hospital Italiano de Buenos Aires (HIBA) durante 2020. Métodos: se realizó un estudio analítico de corte transversal a partir de certificados médicos de defunción de pacientes fallecidos en el ámbito intrahospitalario, evaluando las causas de muerte mal definidas (términos médicos que no aportan información desde el punto de vista clínico y epidemiológico) y las imprecisas (no resultan lo suficientemente específicas como para identificar entidades nosológicas que permitan establecer acciones de prevención y control). Resultados: se analizaron 1030 certificados de defunción, con una proporción de certificados con causa básica de muerte mal definida del 2,3% (n = 24), mientras que en el 17,4% (n = 180) fue imprecisa. No se hallaron diferencias entre la proporción de causas básicas mal definidas y las imprecisas según el día de la semana o período lectivo. Al extender el análisis a todas las causas (básicas, mediatas e inmediatas), la proporción de causas mal definidas fue del 1,6% (n = 40) y la de imprecisas del 51% (n = 1212). Conclusiones: los resultados definen al HIBA como un centro de mediana calidad estadística en el registro de causas de muerte. Se concluye que es necesario mejorarla, para lo que resulta de interés la creación de un plan de capacitación y entrenamiento de los médicos en el grado y el posgrado. (AU)


Introduction: information on causes of death is of great importance both for countries and for health institutions, as it contributes to the evaluation and monitoring of the health status of the population and to the planning of health interventions. The purpose of this study was to evaluate the proportion of ill-defined and imprecise causes of death and its relationship with the day of the week and academic calendar during 2020 at the Hospital Italiano de Buenos Aires. Methods: a cross-sectional study was carried out from data recorded in the death certificates of patients who died in the intrahospital setting, evaluating ill-defined causes of death (medical terms that do not provide clinical or epidemiological information) and imprecise ones (not specific enough to identify nosological entities susceptible to prevention or control). Results: 1030 death certificates were analyzed. The proportion of certificates with ill-defined underlying causes of death was 2.3% (n=24), while 17.4% (n=180) was imprecise. No significant differences were found between the ill-defined and imprecise underlying causes of death and the day of the week and academic calendar. When extending the analysis to all causes (underlying, intermediate, and immediate) the percentage of ill-defined causes was 1.6% (n=40) and 51% (n=1212) was imprecise. Conclusions: results define our hospital as of medium statistical quality on medical death certification. It is concluded that it is necessary to improve the quality of the registry, for which the creation of a training plan for undergraduate and graduate physicians is of interest. (AU)

ABSTRACT

Objetivo: analisar a letalidade materna hospitalar pós-parto segundo risco gestacional e via de parto, no Sistema Único de Saúde, Brasil e macrorregiões, 2010-2019. Métodos: estudo ecológico de série temporal, com dados do Sistema de Informações Hospitalares; a letalidade materna hospitalar pós-parto considerou internações maternas com desfecho "óbito" sobre o total de internações/ano, segundo risco gestacional e via de parto, nas regiões. Resultados: houve 19.158.167 internações para parto e 5.110 óbitos no período analisado; a letalidade materna subiu de 1,1 (2010) para 1,9 óbito/10 mil internações (2019), em gestações de risco habitual após partos vaginais, e reduziu-se de 10,5 (2010) para 7,0 óbitos/10 mil internações (2019) em gestações de alto risco após cesarianas; o Centro-Oeste expressou a maior e o Sul a menor letalidade para gestações de alto risco. Conclusão: a letalidade hospitalar foi maior em gestações de alto risco, com diferenças segundo via de parto e regiões.


Objetivo: analizar la letalidad materna hospitalaria posparto según riesgo gestacional y modalidad de parto por el Sistema Único de Salud en Brasil y regiones entre 2010-2019. Métodos: estudio de serie temporal ecológico con datos del Sistema de Información Hospitalario; la letalidad materna hospitalaria posparto consideró las hospitalizaciones maternas con resultado de óbito, por el total de hospitalizaciones por año. Resultados: hubo 19.158.167 admisiones por parto y 5.110 óbitos en el período; la letalidad materna aumentó de 1,10 (2010) a 1,9 muerte/10.000 (2019) en embarazos de riesgo habitual posparto vaginal y disminuyó de 10,5 a 7,0 muertes/10.000 en embarazos de alto riesgo después de cesáreas; el Centro-Oeste expresó la letalidad más alta y el Sur la más baja para embarazos de alto riesgo. Conclusión: la letalidad hospitalaria fue mayor en los embarazos de alto riesgo, con diferencias según el modo de parto y las regiones de Brasil.


Objective: to analyze in-hospital maternal case fatality ratio in the postpartum period according to pregnancy risks and route of delivery, within the Brazilian National Health System, Brazil and macro-regions, 2010-2019. Methods: this was an ecological time-series study, using data from the Hospital Information System; in-hospital maternal case fatality ratio in the postpartum period took into consideration maternal hospitalizations with outcome 'death' over the total number of hospitalizations per year, according to pregnancy risks and route of delivery, in the regions. Results: there were 19,158,167 hospitalizations for childbirth and 5,110 deaths in the period analyzed; maternal case fatality ratio increased from 1.1 (2010) to 1.9 death/10,000 hospitalizations (2019), in usual-risk pregnancies after vaginal deliveries, and decreased from 10.5 (2010) to 7.0 deaths/10,000 hospitalizations (2019) in high-risk pregnancies after cesarean sections; the Midwest region presented the highest and the South region the lowest case fatality ratio for high-risk pregnancies. Conclusion: in-hospital case fatality ratio was higher for high-risk pregnancies, showing differences according to route of delivery and regions.

Abstract Background Vena cava filter implantation is considered a simple procedure, which can lead to overuse and over-indication. It is nevertheless associated with short and long-term complications. Objectives The goals of this study were to evaluate rates of vena cava filter implantation conducted by Brazil's Unified Public Health System, analyzing in-hospital mortality and migration of patients from other cities seeking medical attention in São Paulo. Methods This study analyzed all vena cava filter procedures conducted from 2008 to 2018 in the city of São Paulo and registered on the public database using a big data system to conduct web scraping of publicly available databases. Results A total of 1324 vena cava filter implantations were analyzed. 60.5% of the patients were female; 61.7% were under 65 years old; 34.07% had registered addresses in other cities or states; and there was a 7.4% in-hospital mortality rate. Conclusions We observed an increase in the rates of use of vena cava filters up to 2010 and a decrease in rates from that year onwards, which coincides with the year that the Food and Drug Administration published a recommendation to better evaluate vena cava filter indications.


Resumo Contexto O implante de filtro de veia cava é considerado um procedimento de baixa complexidade, o que pode resultar em indicação excessiva. No entanto, não é isento de complicações a curto e longo prazo. Objetivos Avaliar as taxas de implantes de filtro de veia cava realizados pelo Sistema Único de Saúde e a origem geográfica e mortalidade intra-hospitalar dos pacientes. Métodos Foi conduzida uma análise em um banco de dados públicos referente às taxas de implantes de filtro de veia cava realizados de 2008 a 2018 na cidade de São Paulo, utilizando o sistema de big data. Resultados Foram analisados 1.324 implantes de filtro de veia cava financiados pelo Sistema Único de Saúde. Identificou-se tendência de aumento da taxa de implantação até 2010 e de redução dos números após esse período. Do total de pacientes, 60,5% eram do sexo feminino; 61,75% tinham menos de 65 anos; e 34,07% possuíam endereço oficial em outra cidade ou estado. A taxa de mortalidade intra-hospitalar foi de 7,4%. Conclusões Observamos aumento das taxas de implante de filtro de veia cava até 2010 e redução das taxas após esse período, o que coincide com o ano em que a organização norte-americana Food and Drug Administration publicou uma recomendação para melhor avaliar as indicações de filtros.

RESUMEN Introducción: la propia asistencia médica provoca, en determinadas situaciones, problemas de salud que pueden llegar a ser importantes para el enfermo. El análisis de la mortalidad es uno de los parámetros utilizados para investigar la seguridad en la realización de procederes de cirugía mayor. Objetivo: determinar los factores asociados a la mortalidad operatoria en cirugías mayores. Materiales y métodos: se realizó un estudio observacional, descriptivo y retrospectivo, de los pacientes que fallecieron tras la realización de una cirugía mayor, en el Hospital Militar Docente Dr. Mario Muñoz Monroy, de Matanzas, en el período comprendido de enero de 2011 a diciembre de 2019. Resultados: la tercera edad aportó 77,3 % de los fallecidos. La hipertensión arterial, diabetes mellitus y cardiopatía isquémica fueron las principales comorbilidades. El abdomen agudo fue el diagnóstico operatorio más frecuente con 98 (58,3 %). Las complicaciones aportaron el 11,9 % de los fallecidos; los eventos adversos, 29,7 %, y por el curso natural de la enfermedad, murió un 58,3 %. El síndrome de disfunción múltiple de órganos y el shock séptico resultaron las principales causas de muerte (62 %). Conclusiones: la mortalidad operatoria estuvo asociada a factores de riesgo como edad avanzada, enfermedades crónicas y cirugía de urgencia. Los eventos adversos elevan la incidencia de mortalidad en cirugía mayor. Las infecciones son la principal causa de mortalidad operatoria (AU).


ABSTRACT Introduction: medical care itself causes, in certain situations, health problems that could be very important for the patient. The mortality analysis is one of the parameters used to study safety performing procedures of major surgery. Objective: to determine the factors associated to operatory mortality in major surgeries. Materials and methods: a retrospective, descriptive and observational study was carried out of the patients who passed away after undergoing a major surgery in the Military Hospital Dr. Mario Munoz Monroy in the period between January 2011 and December 2019. Results: 77.3 % of the deceased were elder people. The main co-morbidities were arterial hypertension, diabetes mellitus and ischemic heart disease. The most frequent surgery diagnosis was acute abdomen with 98 patients (58.3 %). Complications yielded 11.9 % of the deceases, adverse events 29.7 % and 58.3 % died due to the natural course of the disease. The organs multiple dysfunction syndrome and septic shock were the main causes of dead (62 %). Conclusions: operatory mortality was associated to risk factors like advanced age, chronic diseases and emergency surgery. The adverse events increase mortality incidence in major surgery. Infections are the main causes of operatory mortality (AU).

Objetivo: Descrever a letalidade por COVID-19 - hospitalar e em unidade de terapia intensiva (UTI) - em quatro capitais brasileiras, em meses de picos epidêmicos e nos meses anteriores. Métodos: Estudo ecológico com dados mensais de 2020-2021 do Sistema de Informação da Vigilância Epidemiológica da Gripe, em indivíduos com 20 anos ou mais. Foram estimadas letalidade e mortalidade, com intervalos de confiança de 95% (IC95%). Resultados: Em Manaus, a letalidade em UTI nos >59 anos foi menor em dezembro/2020 (80,9%; IC95% 78,4;83,3) e no pico de janeiro/2021 (79,9%; IC95% 77,4;82,5), em comparação ao pico de abril/2020 (88,2%; IC95% 86,1;90,3). Em São Paulo, Curitiba e Porto Alegre, observou-se queda ou estabilidade na letalidade hospitalar e em UTI, em janeiro/2021, em comparação ao mês de referência de 2020. Conclusão: Em janeiro/2021, a letalidade hospitalar e em UTI caiu ou manteve-se estável nas quatro capitais, especialmente em Manaus e durante o pico epidêmico com predomínio da variante Gama.


Objetivo: Describir la tasa de letalidad hospitalaria y en unidad de cuidados intensivos (UCI) por Covid-19 en cuatro capitales brasileñas, durante picos epidémicos y en meses anteriores. Métodos: Estudio ecológico con datos del Sistema de Información de Vigilancia Epidemiológica de la Influenza, durante 2020-2021, en personas de 20 años o más. Se estimaron tasas de mortalidad y letalidad, con intervalos de confianza del 95,0% (IC95%). Resultados: En Manaus, la letalidad en UCI en >59 años fue menor en el pico de enero/2021 (79,9%; IC95% 77,4;82,5) y diciembre/2020 (80,9%; IC95% 78,4;83,3), comparado al pico de abril/2020 (88,2%; IC95% 86,1;90,3). En São Paulo, Curitiba y Porto Alegre hubo disminución o estabilidad para letalidad hospitalaria y en UCI, comparada con la referencia de 2020. Conclusión: En enero/2021, la letalidad hospitalaria y en UCI disminuyó o se mantuvo estable en las cuatro capitales, especialmente en Manaus, y durante el pico epidémico con predominio de la variante Gamma.


Objective: To describe in-hospital and intensive care unit (ICU) case fatality ratios due to COVID-19 in four Brazilian state capitals, during the months of epidemic peaks and previous months. Methods: This was an ecological study using monthly data from the Influenza Epidemiological Surveillance Information System, between 2020-2021, in individuals aged 20 years or older. Case fatality ratio and mortality rate were estimated with 95% confidence intervals (95%CI). Results: In Manaus, the capital city of the state of Amazonas, ICU case fatality ratio among those >59 years old was lower in December/2020 (80.9%; 95%CI 78.4;83.3) and during the peak in January/2021 (79.9%; 95%CI 77.4;82.5), compared to the peak in April/2020 (88.2%; 95%CI 86.1;90.3). In São Paulo, the capital city of the state of São Paulo, Curitiba, the capital city of the state of Paraná, and Porto Alegre, the capital city of the state of Rio Grande do Sul, there was a decrease or stability in ICU and in-hospital case fatality ratio in January/2021, compared to the reference month in 2020. Conclusion: In January/2021, in-hospital and ICU case fatality ratios decreased or remained stable in the four state capitals, especially in Manaus, and during the epidemic peak with the prevalence of the Gamma variant.

Abstract Introduction: Acute aortic dissection (AAD) is a devastating surgical emergency, with high operative mortality. Several scoring algorithms have been used to establish the expected mortality in these patients. Our objective was to define the predictive factors for mortality in our center and to validate the EuroSCORE and Penn classification system. Methods: Patients who underwent surgery for AAD from 2006 to 2016 were retrieved from the institution's database. Preoperative, operative and postoperative variables were collected. Observed and expected mortality was calculated by EuroSCORE. Logistic regression analysis and Cox regression analysis were performed to find predictors of operative mortality and survival, respectively. The receiver operating characteristic (ROC) curves were plotted for logistic EuroSCORE, and the area under the ROC curve (AUC) was calculated. Results: 87 patients (27.6% female) underwent surgery for AAD. The mean age was 58.6±9.7 years. Expected and observed operative mortality was 25.8±15.1% and 20.7%, respectively. Penn Aa, Ab and Abc shared similar observed/expected (O/E) mortality ratio. The only independent predictor of operative mortality (OR: 3.63; 95% CI: 1.19-11.09) and survival (HR: 2.6; 95% CI: 1.5-4.8) was female gender. EuroSCORE showed a very poor prediction capacity, with an AUC=0.566. Conclusion: Female gender was the only independent predictor of operative mortality and survival in our institution. EuroSCORE is a poor scoring algorithm to predict mortality in AAD, but with consistent results for Penn Aa, Ab and Abc.

ABSTRACT Introduction : During the COVID-19 pandemic, endoscopic procedures are associated with a high risk of SARS-CoV-2 infection. However, in cases of upper gastrointestinal bleeding (UGIB), priority should be given to an early endoscopy. Objective : The main objective was to compare the time since arrival at the hospital and the performance of the endoscopy between both groups. Materials and methods : We performed a retrospective study. Data contains information of patients who attended to the hospital with UGIB and underwent an endoscopy between October 19th, 2019 and June 6th, 2020. Patients were divided into 2 phases: pre-pandemic and pandemic. The time between arrival at the hospital and the performance of the endoscopy in both phases were compared as well as other indicators such hospital stay and in-hospital mortality. Results : With information from 219 patients, the median age was 69 years. 154 and 65 endoscopies were performed in pre-pandemic and pandemic phase, respectively. The time between arrival at the hospital and the performance of the endoscopy was significantly longer during the pandemic (10.00 vs. 13.08 hours, p-value = 0.019). Nevertheless, there were no significant differences in hospital stay or mortality. Conclusion : The management of patients with UGIB during the COVID-19 pandemic is complex and requires the application of clinical judgment to decide the best timing to perform an endoscopy without affecting patient care.


RESUMEN Introducción : Durante la pandemia de COVID-19, los procedimientos endoscópicos se asocian con un alto riesgo de infección por SARS-CoV-2. Sin embargo, en casos de hemorragia digestiva alta (HDA), se debe dar prioridad a una endoscopia precoz. Objetivo : El objetivo principal fue comparar el tiempo transcurrido desde la llegada al hospital y la realización de la endoscopia entre ambos grupos. Materiales y métodos : Realizamos un estudio retrospectivo. Los datos contienen información de pacientes que acudieron al hospital con HDA y fueron sometidos a endoscopia entre el 19 de octubre de 2019 y el 6 de junio de 2020. Los pacientes se dividieron en 2 fases: prepandémica y pandémica. Se comparó el tiempo transcurrido entre la llegada al hospital y la realización de la endoscopia en ambas fases, así como otros indicadores como la estancia hospitalaria y la mortalidad intrahospitalaria. Resultados : Con información de 219 pacientes, la mediana de edad fue de 69 años. Se realizaron 154 y 65 endoscopias en fase prepandémica y pandémica, respectivamente. El tiempo entre la llegada al hospital y la realización de la endoscopia fue significativamente mayor durante la pandemia (10,00 frente a 13,08 horas, valor de p = 0,019). Sin embargo, no hubo diferencias significativas en la estancia hospitalaria ni en la mortalidad. Conclusión : El manejo de pacientes con HDA durante la pandemia de COVID-19 es complejo y requiere la aplicación del juicio clínico para decidir el mejor momento para realizar una endoscopia sin afectar la atención del paciente.

Objective: To describe and analyze data on self-injurious behavior (SIB) and related mortality in children under 10 years old in Brazil. Methods: A descriptive study was performed using secondary public health care data extracted from the Hospital Information System (Sistema de Informações Hospitalares, SIH) and Mortality Information System (Sistema de Informações sobre Mortalidade, SIM) in Brazil. The databases are available for online access at http://datasus.saude.gov.br/. Results: In Brazil, according to SIH data, 11,312 hospitalizations of patients under 10 years of age were recorded from 1998 to 2018 as resulting from SIB (ICD-10 X60-X84 codes). Of these, 65 resulted in death. According to the SIM, from 1996 to 2016, 91 deaths related to SIB were recorded, 81 (89%) in children aged 5 to 9 years, nine (9.9%) in children aged 1 to 4 years, and one (1.1%) in a child below 1 year of age. Conclusion: These results highlight the relevance of creating measures to better understand SIB and related mortality in this age group. They also reveal the vulnerability of children in Brazil and warrant further studies to address these issues.

Abstract Background: Data on heart failure (HF) epidemiology in less developed areas of Brazil are scarce. Objective: Our aim was to determine the HF morbidity and mortality in Paraiba and Brazil and its 10-year trends. Methods: A retrospective search was conducted from 2008 to 2017 using the DATASUS database and included patients ≥ 15 years old with a primary diagnosis of HF. Data on in-hospital and population morbidity and mortality were collected and stratified by year, gender and age. Pearson correlation and linear-by-linear association test for trends were calculated, with a level of significance of 5%. Results: From 2008 to 2017, HF admissions decreased 62% (p = 0.004) in Paraiba and 34% (p = 0.004) in Brazil. The in-hospital mortality rate increased in Paraiba and Brazil [65.1% (p = 0.006) and 30.1% (p = 0.003), respectively], but the absolute in-hospital mortality had a significant decrease only in Paraiba [37.5% (p = 0.013)], which was maintained after age stratification, except for groups 15-19, 60-69 and > 80 years. It was observed an increase in the hospital stay [44% (p = 0.004) in Paraiba and 12.3% (p = 0.004) in Brazil]. From 2008 to 2015, mortality rate for HF in the population decreased 10.7% (p = 0.047) in Paraiba and 7.7% (p = 0.017) in Brazil. Conclusions: Although HF mortality rate has been decreasing in Paraiba and Brazil, an increase in the in-hospital mortality rate and length of stay for HF has been observed. Hospital-based clinical studies should be performed to identify the causes for these trends of increase.


Resumo Fundamento: Dados sobre a epidemiologia da insuficiência cardíaca (IC) em áreas pouco desenvolvidas são escassos. Objetivos: Nosso objetivo foi determinar a morbidade e a mortalidade por IC na Paraíba e no Brasil, e sua tendência em dez anos. Métodos: Realizou-se uma busca retrospectiva de 2008 a 2017 utilizando-se o banco de dados do DATASUS incluindo pacientes com idade ≥ 15 anos, com diagnóstico primário de IC. Os dados da morbimortalidade por IC foram coletados e estratificados por ano, sexo e idade. Foram realizados correlação de Pearson e teste para tendências de Mantel-Haenzsel. Um nível de 5% foi definido como estatisticamente significativo. Resultados: De 2008 a 2017, as internações por IC diminuíram 62% (p = 0,004) na Paraíba, e 34% (p = 0,004) no Brasil. A taxa de mortalidade hospitalar aumentou na Paraíba e no Brasil [65,1% (p = 0,006) e 30,1% (p = 0,003), respectivamente], mas a mortalidade hospitalar em números absolutos apresentou uma diminuição significativa somente na Paraíba [37,5% (p = 0,013)], o que foi mantido após a estratificação por idade, exceto para os grupos 15-19, 60-69 e > 80 anos. Observou-se um aumento no período de internação [44% (p = 0,004) na Paraíba e 12,3% (p = 0,004) no Brasil]. De 2008 a 2015, a taxa de mortalidade por IC na população diminuiu 10,7% na Paraíba (p = 0,047) e 7,7% (p = 0,017) no Brasil. Conclusões: Apesar de a taxa de mortalidade por IC estar diminuindo na Paraíba e no Brasil, observou-se um aumento na taxa de mortalidade hospitalar e na duração da internação por IC. Devem ser realizados estudos clínicos em hospitais para serem identificadas as causas dessa tendência de aumento.

Resumo No contexto de crise e restrições de recursos é razoável supor o agravamento de fragilidades do Sistema Único de Saúde (SUS), como desigualdades regionais, subfinanciamento e problemas na qualidade do cuidado. Este estudo explorou a aplicação de indicadores de acesso e efetividade, facilmente compreensíveis e calculados, passíveis de refletir a crise na rede hospitalar. Cinco indicadores extraídos do Sistema de Informações Hospitalares, relativos ao Brasil e a estados da Região Sudeste, foram analisados no período de 2009-2018: internações resultantes em morte; internações cirúrgicas resultantes em morte; cirurgias eletivas no total das internações cirúrgicas; próteses de quadril na população de idosos; e angioplastias na população de 20 anos ou mais. Utilizaram-se gráficos de controle estatístico para a comparação dos indicadores entre estados, antes e a partir de 2014. No Brasil, as mortes hospitalares tiveram um leve crescimento enquanto que as mortes cirúrgicas uma queda; as cirurgias eletivas e próteses de quadril também diminuíram. No Sudeste, o Rio de Janeiro apresentou os piores resultados, em especial a queda de cirurgias eletivas. Os resultados ilustram o potencial dos indicadores para monitorar efeitos da crise sobre o cuidado hospitalar.


Abstract In the context of crisis and resource constraints, it is reasonable to assume the deteriorated weaknesses of the Unified Health System (SUS), such as regional inequalities, underfinancing, and care quality issues. This study explored the application of easily comprehensible and calculated access and effectiveness indicators that could reflect the hospital network crisis. Five indicators extracted from the Hospital Information System, related to Brazil and states of the Southeastern region, were analyzed in the 2009-2018 period: hospitalizations resulting in death; surgical hospitalizations resulting in death; elective surgeries in the total of surgical hospitalizations; hip prostheses in the senior population; and angioplasties in the population aged 20 years and over. Statistical control charts were used to compare indicators between states, before and from 2014. In Brazil, overall hospital deaths had a slight increase while surgical deaths declined; elective surgeries and hipprosthesis also decreased. In Southeastern Brazil, Rio de Janeiro was the worst performer, especially the decrease of the elective surgeries. The results illustrate the potential of indicators to monitor crisis effects on hospital care.

Background: With the aim to inform end of life public policies, the place of death in Chile, its trends and associated factors were analysed. Materials and Methods: A cross sectional using publically available death database from 1990 to 2014 was conducted. The proportion of hospital deaths was selected as the main outcome. A logistic regression was used to assess the association between place of death, age, and main diagnosis at death. Also, a Prais-Winsten regression and a Chi2 test were used to assess a time series and regional analysis, respectively. Results: 2,063,615 deaths were analysed. Overall, deaths 898,871 (43.6%) occurred at hospital. Those who died over 85 years (OR 2,52 IC95% 2.49-2.55) and those who died from cancer (OR 2.43 IC95% 2.42-2.45) had higher risk for dying outside de hospital. For the general population and those who die form cancer, there is no evidence for an increase or decrease trend in the proportion of hospital deaths over time (p = 0,75 and p = 0.68, respectively). However, there is an increase of the proportion of hospital deaths in those who died over 85 years (p < 0.001,27% in 1990 to 32% in 2014). Also, there are geographic differences between country regions (p < 0.001) (Eg. Magallanes 52.9%). Conclusions: The proportion of hospital deaths has been stable over time in the general population and has increased in those over 85 years.

ABSTRACT

OBJETIVO: Avaliar fatores de risco para mortalidade em pacientes idosos com internação em uma unidade de terapia intensiva (UTI). MÉTODOS: Foram estudados pacientes idosos admitidos em UTI, em um estudo de coorte histórica. Foi realizada amostragem de conveniência de todos os pacientes com 60 anos ou mais, internados nos 20 leitos de uma UTI de um hospital de alta complexidade. Foi realizada análise de regressão de Cox para estimar os fatores de risco associados à mortalidade. O nível de significância utilizado foi de 5%. RESULTADOS: A proporção de pacientes idosos representou 45% das admissões em UTI no período. A mortalidade hospitalar foi de 38,4%. A variável pós-operatório de cirurgia de urgência e os escores Acute Physiology and Chronic Health Evaluation (APACHE II) e Sequential Organ Failure Assessment (SOFA) da admissão foram identificados como fatores de risco independentes para mortalidade hospitalar. CONCLUSÃO: Os fatores de risco associados à mortalidade foram os escores APACHE II e SOFA da admissão e pós-operatório de cirurgia de urgência.


OBJECTIVE: To evaluate risk factors for mortality in geriatric patients admitted to an intensive care unit (ICU). METHODS: We studied older patients admitted to the ICU in a historical cohort study. Convenience sampling was performed for all patients aged 60 years or older, admitted to the 20 beds of an ICU of a highly complex hospital. Cox regression analysis was performed to estimate risk factors associated with mortality. The significance level used was 5%. RESULTS: The proportion of geriatric patients represented 45% of ICU admissions in the period. In-hospital mortality was 38.4%. The postoperative emergency surgery variable and the Acute Physiology and Chronic Health Evaluation (APACHE II) and Sequential Organ Failure Assessment (SOFA) admission scores were identified as independent risk factors for in-hospital mortality. CONCLUSION: Risk factors associated with mortality were APACHE II and SOFA scores for admission and postoperative emergency surgery.

ABSTRACT OBJECTIVE Assess the magnitude and trend of hospitalization rates due to traumatic injuries in intensive care units (ICU) in Brazil from 1998 to 2015. METHODS This is an ecological time-series study that analyzed data from the Hospital Information System. A trend analysis of hospitalization rates was performed according to diagnosis, sex and age using generalized linear regression models and Prais-Winsten estimation. RESULTS Rates were higher among male patients, but increased hospitalization due to trauma among female patients influenced the ratio between both sexes. Falls and transport accidents were the most frequent causes of trauma. The average annual growth was 3.6% in ICU trauma hospitalization rates in Brazil, the highest growth was reported in the North region (8%; 95%CI 6.4-9.6), among women (5.4%; 95%CI 4.5-6.3), and among people aged 60 years and older (5.5%; 95%CI, 4.7-6.3). The most frequent causes of trauma are falls (4.5%; 95%CI 3.5-5.5) and care complications (5.4%; 95%CI 4.5-6.3). On the other hand, the annual hospital mortality rate due to trauma in ICU is 1.7% lower, on average (95%CI 2.1-1.3). CONCLUSION An increase in ICU hospitalization rate due to trauma in Brazil may be the result of some factors, such as an increasing number of accidents and cases of violence, the implementation of pre-hospital care, and improved access to care, with more beds in ICU. In addition, population aging is another factor, as a greater increase in hospitalization was observed among people aged 60 years and older.


RESUMO OBJETIVO Analisar a magnitude e a tendência das taxas de internação por lesões traumáticas em unidades de terapia intensiva (UTI) no Brasil de 1998 a 2015. MÉTODOS Estudo ecológico de séries temporais com dados do Sistema de Informações Hospitalares. A análise de tendência das taxas de internação segundo diagnóstico, sexo e idade foi realizada por modelos de regressão linear generalizada e procedimento de Prais-Winsten. RESULTADOS As taxas foram mais elevadas para os homens, mas o crescimento das internações por trauma para as mulheres influenciou a razão das taxas entre os sexos. As quedas e os acidentes de transporte foram as causas mais frequentes dos traumas. Houve aumento de 3,6% ao ano, em média, nas taxas de internação por trauma em UTI no Brasil, maior na região Norte (8%; IC95% 6,4-9,6), entre as mulheres (5,4%; IC95% 4,5-6,3) e pessoas com 60 anos ou mais (5,5%; IC95% 4,7-6,3). Entre as causas do trauma, as quedas (4,5%; IC95% 3,5-5,5) e complicações assistenciais (5,4%; IC95% 4,5-6,3) foram as mais importantes. Por outro lado, as taxas de mortalidade hospitalar por trauma em UTI declinaram 1,7% ao ano, em média (IC95% 2,1-1,3). CONCLUSÃO O aumento das internações por trauma em UTI no Brasil pode ser resultado de alguns fatores, como a ocorrência crescente de acidentes e violências, a implementação do atendimento pré-hospitalar e também a melhoria no acesso à assistência, com maior número de leitos em UTI. Soma-se a isso o envelhecimento da população, pois observou-se aumento mais acentuado das internações em pessoas com 60 anos de idade ou mais.

RESUMO: Objetivo: Analisar a tendência das taxas de internação e de mortalidade de idosos por condições sensíveis à atenção primária (CSAP) no estado de Santa Catarina, Brasil, no período de 2008 a 2015. Método: Estudo ecológico de série temporal, com dados do Sistema de Informações Hospitalares (SIH) do Sistema Único de Saúde (SUS), utilizando as autorizações de internações hospitalares como fonte de dados, no período de 2008 a 2015, em Santa Catarina. Os dados foram analisados por sexo e idade, estratificada em duas faixas etárias: 60a 79 anos e 80 anos ou mais. Foram realizadas a padronização das taxas por idade através do método direto e a análise estatística por meio de regressão linear segmentada (joinpoint regression). Resultados: Asinternações por condições sensíveis representaram 41% do total de internações de idosos em 2008 e 32% em 2015. Avariação anual da taxa comportou-se de maneira decrescente [-4,6^ IC (-5,7; -3,6)]. As causas mais prevalentes foram: insuficiência cardíaca, doença pulmonar obstrutiva crônica e doenças cerebrovasculares. As taxas de mortalidade das internações apresentaram redução, independentemente do sexo e na faixa etária até 80 anos, representando variação anual de menos 2,4%, 2,1% para os idosos e 2,7% para as idosas. Conclusão: Por meio da análise do indicador CSAP, que avalia a qualidade da assistência e a efetividade dos cuidados prestados na atenção primária, os resultados desta pesquisa evidenciaram queda nas taxas de internação de idosos, independentemente do sexo e das faixas etárias estabelecidas no estudo.


ABSTRACT: Objective: To analyze the trends in hospitalization and mortality rates for ambulatory care sensitive conditions (ACSC) among older adults in the state of Santa Catarina, Brazil, from 2008 to 2015. Method: Ecological time-series study based on data from the Hospital Information System (Sistema de Informações Hospitalares - SIH) of the public health system (Sistema Único de Saúde - SUS), using hospital admission authorization forms as data source, from 2008 to 2015, in Santa Catarina. Data were analyzed by gender and age, which was stratified into two age groups: 60 to 79 years and 80 years or older. We standardized age rates using the direct method and statistical analysis using segmented linear regression (joinpoint regression). Results: Sensitive conditions led to 41% of total hospitalizations of older adults in 2008 and 32% in 2015. The annual rate variation decreased [-4.6^ 95%CI (-5.7; -3.6)]. The most prevalent causes were: heart failure, chronic obstructive pulmonary disease, and cerebrovascular diseases. Hospitalization mortality rates dropped, regardless of gender and in the age group up to 80 years, representing an annual variation of -2.4%, with -2.1% for males and -2.7% for females. Conclusion: Analyzing the ACSC indicator, which evaluates the quality and effectiveness of primary care, the results of this research revealed a reduction in hospitalization rates among older adults, regardless of gender and age groups established in this study.

ABSTRACT Background Candidemia is the most frequent invasive fungal disease in hospitalized patients, and is associated with high mortality rates. The main objective of this study was to evaluate changes in the epidemiology of candidemia at a tertiary care hospital in a 21-year period. Methods We evaluated all episodes of candidemia diagnosed between 1996 and 2016 at a University-affiliated tertiary care hospital in Brazil. We arbitrarily divided the study period in 3: 1996-2002 (period 1), 2003-2009 (period 2) and 2010-2016 (period 3). Incidence rates were calculated using hospital admissions as denominator. Results We observed 331 episodes of candidemia. The incidence was 1.30 episodes per 1000 admissions, with no significant change over time. Candida albicans (37.5%), C. tropicalis (28.1%), C. parapsilosis (18.4%) and C. glabrata (6.9%) were the most frequent species. The proportion of patients receiving treatment increased (65.5%, 79.4% and 74.7% in periods 1, 2 and 3, respectively, p= 0.04), and the median time from candidemia to treatment initiation decreased from 4 days in period 1 (range 0-32 days) to 2 days in period 2 (range 0-33 days) and 2 days in period 3 (range 0-14 days, p< 0.001). We observed a significant decrease in the use of deoxycholate amphotericin B (47.4%, 14.8% and 11.9%), and an increase in the use of echinocandins (0%, 2.8% and 49.1%; p< 0.001). The APACHE II score increased over time (median 16, 17.5, and 22, p< 0.001). The overall 30-day mortality was 58.9%, and did not change significantly over the study period. Conclusions There was an improvement in patient care, with an increase in the proportion of patients receiving treatment and a decrease in the time to treatment initiation, but no improvement in the outcome, possibly because the proportion of sicker patients increased over time.

ABSTRACT Objective: To present official longitudinal data on the impact of asthma in Brazil between 2008 and 2013. Methods: This was a descriptive study of data collected between 2008 and 2013 from an official Brazilian national database, including data on asthma-related number of hospitalizations, mortality, and hospitalization costs. A geographical subanalysis was also performed. Results: In 2013, 2,047 people died from asthma in Brazil (5 deaths/day), with more than 120,000 asthma-related hospitalizations. During the whole study period, the absolute number of asthma-related deaths and of hospitalizations decreased by 10% and 36%, respectively. However, the in-hospital mortality rate increased by approximately 25% in that period. The geographic subanalysis showed that the northern/northeastern and southeastern regions had the highest asthma-related hospitalization and in-hospital mortality rates, respectively. An analysis of the states representative of the regions of Brazil revealed discrepancies between the numbers of asthma-related hospitalizations and asthma-related in-hospital mortality rates. During the study period, the cost of asthma-related hospitalizations to the public health care system was US$ 170 million. Conclusions: Although the numbers of asthma-related deaths and hospital admissions in Brazil have been decreasing since 2009, the absolute numbers are still high, resulting in elevated direct and indirect costs for the society. This shows the relevance of the burden of asthma in middle-income countries.


RESUMO Objetivo: Apresentar dados longitudinais oficiais sobre o impacto da asma no Brasil entre 2008 e 2013. Métodos: Estudo descritivo de dados extraídos de um banco de dados do governo brasileiro entre 2008 e 2013, no qual foram analisados as hospitalizações e óbitos por asma, bem como o custo das hospitalizações. Foi também realizada uma subanálise geográfica. Resultados: Em 2013, 2.047 pessoas morreram de asma no Brasil (5 óbitos/dia), com mais de 120.000 hospitalizações por asma. Durante o período de estudo, o número absoluto de óbitos e hospitalizações por asma diminuiu 10% e 36%, respectivamente. No entanto, a taxa de mortalidade hospitalar aumentou aproximadamente 25%. A subanálise geográfica mostrou que as regiões Norte/Nordeste e Sudeste apresentaram as maiores taxas de hospitalização e mortalidade hospitalar por asma, respectivamente. A análise dos estados representativos de cada região mostrou discrepâncias entre as hospitalizações por asma e as taxas de mortalidade hospitalar por asma. Durante o período de estudo, as hospitalizações por asma custaram US$ 170 milhões ao sistema público de saúde. Conclusões: Embora os óbitos e hospitalizações por asma no Brasil estejam diminuindo desde 2009, os números absolutos ainda são altos, com elevados custos diretos e indiretos para a sociedade, o que mostra a relevância do impacto da asma em países de renda média.

ABSTRACT

Esta tesis busca promover una mirada crítica acerca de los procesos de internación y muerte en los espacios manicomiales y sobre las tensiones en la implementación de las políticas públicas en salud mental. Consiste en un estudio de casos que tiene el objetivo de describir y caracterizar las particularidades que adquieren los recorridos de internación de las mujeres fallecidas en el período 1957-1971 en el Asilo de Lomas, actual Hospital Interzonal J. A. Esteves. La selección de dicho período obedece a que en esos quince años falleció la mitad de las personas fallecidas allí en todo el siglo (2088). El análisis se realizó a partir de historias clínicas (HC) y entrevistas a informantes clave. Asimismo se efectuó un relevamiento bibliográfico y se construyó una base de datos con 372 HC a partir de la cual se elaboró un perfil socioepidemiológico de las mujeres fallecidas. Se reconstruyeron ocho recorridos de internación y se los puso en relación con la historia del hospital, las discusiones en salud mental, las políticas en salud mental y el contexto económico político de la época en nuestro país. Este trabajo se enmarca dentro de la corriente de pensamiento de la Medicina Social Latinoamericana que estudia los procesos salud-enfermedad atención (PSEA) desde una articulación jerarquizada entre los niveles singular, institucional y económico político (Stolkiner, 1994; Castellanos, 1990). Como resultado se destaca la cantidad de registros de procedimientos clínicos que sugieren condiciones de higiene y cuidado clínico deficientes. ABSTRACT

BACKGROUND/AIMS: Data on the epidemiology of alcoholic cirrhosis, especially in Asian countries, are limited. We compared the temporal evolution of patterns of alcoholic and nonalcoholic cirrhosis over the last decade. METHODS: We retrospectively examined the inpatient datasets of five referral centers during 2002 and 2011. The study included patients who were admitted due to specific complications of liver cirrhosis. We compared the causes of hospital admissions and in-hospital deaths between patients with alcoholic and nonalcoholic cirrhosis. RESULTS: Among the included 2,799 hospitalizations (2,165 patients), 1,496 (1,143 patients) were from 2002, and 1,303 (1,022 patients) were from 2011. Over time, there was a reduction in the rate of hepatic encephalopathy (HE) as a cause of hospitalization and an increase in the rate of hepatocellular carcinoma. Deaths that were attributable to HE or spontaneous bacterial peritonitis (SBP) significantly decreased, whereas those due to hepatorenal syndrome (HRS) significantly increased over time in patients with alcoholic cirrhosis. However, in patients with nonalcoholic cirrhosis, hepatic failure and HRS remained the principal causes of in-hospital death during both time periods. CONCLUSIONS: The major causes of in-hospital deaths have evolved from acute cirrhotic complications, including HE or SBP to HRS in alcoholic cirrhosis, whereas those have remained unchanged in nonalcoholic cirrhosis during the last decade.
